Full Time - Salaried Position
Monday - Friday 8:00 a.m - 4:30 p.m.
Pay Range: $95,000 - $99,000/year

SUMMARY

The purpose of this position is to provide an organized and structured Plan of Nursing Care for the residents at all times. In the absence of the Director of Nursing, the Assistant Director of Nursing will assume the duties of the Director of Nursing as outlined in the job description.

Job Duties:

  1. Development and maintenance of nursing service policies and procedures to implement the program of care.
  2. Oversight of Advanced Directives for all nursing residents with completion of the 72 hour advanced directive audit.
  3. Determine in-service needs of nursing staff with recommendations from nursing management.
  4. Keep current license renewals of all nursing employees.
  5. Completion of quality indicator reviews (QIs 17, 18, 19, 20) monthly.
  6. Assist DON with annual competencies for nursing associates.
  7. Presentation of Accident Prevention, Disaster, elopement, and Symbols for Orientation.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

1. Assist DON with daily clinical rounds.

2. Reviews 24-hour report, personally check residents and documentation.

3. Interviews potential resident with assistance by Social Services Department, as needed.

4. Assist with admissions by completing medical assessment forms (i.e. 3871)

5. Responds to customer complaints and reports to Administrator and Director of Nursing.

6. Assist DON with annual competences for Nursing Associates.

7. Assist in completing MDS in absence of MDS Coordinator.

8. Maintains communication with Medical Director and attending physicians, as required by administrative staff and Medical Director.

9. Provide daily direction to license staff and counsel as needed.

10. Receive daily report from Team Leaders and report resident and staff issues to Director of Nursing and the Administrator.

11. Monitor staff assignments and scheduling to assure fair and equitable assignments.

12. Conducts interviews for nursing positions.

13. Coordinate and chair the fall, hydration, quality indicator, and medication review committees with completion of meeting minutes.

14. Determine in-service needs of Nursing Staff with recommendations from Nursing Management.

15. Monitor performance of licensed staff and assist with evaluations.

16. Participate in on-call supervision of the facility.

18. Monitor compliance with infection control standards.

19. Review and log incident reports into the incident tracking system.

 

20. Reviews job descriptions annually, along with the Director of Nursing and Administrator, notifying Human Resources Department of revisions.

21. Monitors I & O records and follow up with residents and staff intervention.

22. Assist with revision and development of nursing department policies and procedures.

23. Participate in survey process with regulatory agencies.

SAFETY

 

1. Report equipment malfunctions to appropriate staff and completes work orders when indicated.

2. Identify and remove safety hazards/devices and reports to appropriate personnel.

3. Assure a safe, clean, and clutter free environment.

4. Utilize standard precautions by wearing protective equipment (goggles, gowns, gloves) when indicated, and uses correct hand washing technique.

5. Follow facility fire and safety policies.

6. Participate in the implementation and maintenance of infection control program for monitoring communicable and/or infectious diseases including TB and blood borne pathogens among the residents and personnel.

 

INFECTION CONTROL

 

1. Ensures proper hand washing technique and infection control measures as follows.

2. Assists in established isolation precautions and procedures.

3. Monitors all poisons (medications, creams, lotions, cleaning solutions) that are locked and out of reach of the residents.

4. Follow established safety precautions in the performance of all duties.

5. Participate in fire and disaster drills.

6. Follow established smoking regulations and report all violations.

 

COMPUTER Enter, adjust and retrieve data using a microcomputer or personal computer and keyboard.

 

SUPERVISORY RESPONSIBILITIES

Supervises all Nursing staff in conjunction with the Director of Nurses and when directed by the Administrator.

EDUCATION and/or EXPERIENCE

RN and two years of nursing management experience.

 

 

LANGUAGE SKILLS

Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization.

 

MATHEMATICAL SKILLS

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

 

 

REASONING ABILITY

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

 

CERTIFICATES, LICENSES, REGISTRATIONS

Current licensure in the state of Maryland as a Registered Nurse.

Infection Control Nurse is responsible for the planning, organization, development, implementation, direction, and management the Infection Prevention in the facility. Infection Control Nurse will work to identify, investigate, monitor, and report healthcare-associated infections and communicable diseases in the facility. Infection Control Nurse collaborates with other facility staff members as well as outside Infection Control entities to create, educate, and sustain Infection Prevention and Control strategies for use in the facility. The Infection Control Nurse will conduct ongoing staff development, training and Quality Assurance/Performance Improvement monitoring to insure adherence with the organizational standards, professional guidelines, state, and federal regulations.

 

The Quality Assurance/Infection Control Coordinator is responsible for planning and overseeing a comprehensive quality assurance program to ensure compliance with state and federal standards. Coordinator performs a variety of standards compliance functions including those related to resident review, infection control, in-service training, and quality assurance. These responsibilities include development, implementation, analysis and reporting for various regulatory and plan based performance studies. Assigned work is performed under the supervision of the DON/ADON with permission to exercise independent judgment in carrying out specific duties. Coordinator is expected to work as part of a team in promoting excellence in nursing home standards.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  1. Coordinates and chairs the monthly and quarterly Quality Assurance meetings. Record meeting minutes accordingly and assure the ombudsman, resident, and family council receive a copy of the minutes.
  2. Participates in the monthly and quarterly Quality Indicator/ Quality Measures meetings. Complete assigned quality indicator reviews.
  3. Participates in the Fall and Hydration Committee by analyzing trends and patterns and creating an action plan as needed.
  4. Completes the transcription, lab, routine lab, Coumadin, QA chart, and weight audits monthly. Completes Diabetic Audits quarterly. May delegate audits accordingly.
  5. Coordinates and assures a Bladder Program is established for every resident that would benefit from the program. Complete an initial Toilet/Bladder Assessment for all residents to assess their bladder function and assure needs are met. Another bladder assessment will be completed yearly and if the resident has a change in bladder habits to assure a program is implemented.
  6. Track and analyze medication errors and clinical incidents. Assure appropriate follow up and discipline is completed. Create a threshold for each individual and discipline as needed with DON or ADON.
  7. Assure all residents with a restraint have a yearly consent signed by their family and have progress notes signed by the PCP every 3 months. The goal is to have the least restrictive device and to be as restraint free as possible.
  8. Complete psychotropic logs monthly and complete semi-annual DISCUS and Obra forms from residents on anti-psychotics. Monitor for EPS symptoms and follow-up with physician if indicated. The goal is to reduce therapy at least every 3 months, if not; a progress note is to be completed with a physician evaluation.
  9. Track all medicines that require lab drug levels to be drawn monthly. Complete a lab audit monthly to assure all abnormal drug levels had the appropriate interventions implemented.
  10. Track IVs, Foleys (Assure each resident has a diagnosis and progress note from the urologist or PCP), Hospice, Care and Comfort, Oxygen, Diabetics, and residents with feeding tubes monthly. Analyze the data and action plan any trend or pattern identified, this includes root cause analysis if needed.
  11. Complete mortality/morbidity reviews on each deceased resident to determine unexpected vs. expected death. If unexpected, have the Medical Director complete the Medical Director Review From for unexpected deaths with interventions as deemed necessary.
  12. Complete unplanned transfers for residents that are transported to the hospital, etc… unexpectedly to determine avoidable vs. unavoidable status. Log all unplanned transfers on unplanned transfer log. If the transfer is considered avoidable, education/disciplinary process is indicated.
  13. Performs other administrative duties as required such as completing medical forms, reports, audits, as necessary.
  14. Collaborates and provides advisement to all departments regarding associate educational/training needs.
  15. Conducts quality assurance projects as they relate to survey readiness-
    1. Dining experience audits
    2. Call bell audits
    3. Safety checks/hazard checks
    4. Pharmacy medication reviews
    5. Change in condition audits
    6. Kardex audits
    7. GNA peri-care audits
    8. Assures housekeeping, maintenance, and dietary completes audits for resident safety. Analyzes data and action plans as needed
  16. Advises the DON/ADON regarding potential resident care or life problems.
  17. Participates in the monthly safety committee.
  18. Participate and coordinate pain management committee.
  19. Complete action plans as needed. Assign duties to appropriate staff and ensure follow up.
  20. Coordinate a Root Cause Analysis Program for sentinel events.
  21. Complete and update policies and procedures as needed.
  22. Complete all other assigned audits as scheduled.
  23. Infection control: Plans and maintains an Infection Control Program designed to provide a safe, sanitary and comfortable environment. Ensures all infection control activities are completed by Federal and State standards with follow up on any suspected outbreaks with appropriate measures taken.
    1. Analyze and interpret infection control data
    2. Investigate and survey suspected outbreaks of infection
    3. Plan, implement, and evaluate infection prevention and control measures
  24. Provides managerial oversight of all infection control activities including policy setting, education, and prevention/control activities.
    1. Educate individuals about infection risk, prevention, and control
    2. Develop and revise infection control policies and procedures
    3. Manage infection prevention and control activities
    4. Provide consultation on infection risk assessment, prevention, and control strategies
    5. Monitor the cleaning of the glucometers
  25. Read 24 hour report daily – use this info as a double check for infection control issues.
  26. Rounds on each floor to check boards for infections.
  27. Review all charts of residents with infections.
  28. Assure the wet task are filled for the nursing carts for daily cleaning twice a week – may delegate.
  29. Monitor and coordinate the nightshift cleaning schedule for infection control measures.
  30. Review lab reports daily and put on isolation when needed.
  31. Order labs for residents who need to come off of isolation.
  32. Obtain the vaccination records of new admissions and completes the armbands for communication to the hospital
  33. Map all infections monthly for trends and patterns.
  34. Distribute new updates on flu activity, TB changes, etc… to physicians, CRNP, and fellow associates as needed.
  35. Within 72 hours of a new admission, check to assure all vaccines has been given. If vaccines have not been given, schedule vaccines.
  36. When an outbreak occurs, line listing to be completed daily, along with notification to health department, etc…
  37. Complete yearly flu vaccine administration with assistance from licensed staff. QA will be aware of all of the consents, allergies, and potential complications during this process.
  38. Files and analyzes 11/7 responsibility, emergency cart ,cleaning schedules, glucometer checks, alarm checks, and lift battery checks and requires action plan as needed.
  39. Other duties as assigned.
