SUMMARY:

The primary purpose of a Certified Medication Aide is to assist in the administering of medications to residents as ordered by the attending physician, under the direction of the attending physician, the Nurse Supervisor or the Charge Nurse and the Director of Nursing.  The administration of medications shall be in accordance with established nursing standards, the policies, procedures, and practices of Fahrney-Keedy, and the requirements of the State. 

 

DELEGATION OF AUTHORITY:

As a Certified Medication Aide, you are delegated the administrative authority, responsibilities, and accountability necessary for carrying out your assigned duties.

 

EMPLOYMENT AT WILL:

Employment with Fahrney-Keedy is “at will.”  Either the associate or Fahrney-Keedy may terminate the employment relationship for any reason at any time without notice, regardless of the length of employment or the granting of benefits of any kind, including but not limited to benefits which provide for vesting based on length of service. 

 

CONFIDENTILITY:

Fahrney-Keedy associates recognize that Fahrney-Keedy has and will have information regarding financial, personal, and medical affairs and other vital information (collectively, “information”) which are valuable, special, and unique assets of Fahrney-Keedy.  Associates agree that they will not at any time or in any manner solicit, collect, or divulge any Information to any third party without the prior written consent of Fahrney-Keedy.  This information is Company property which may not be disclosed outside the facility unless properly released for publication.  Associates will protect the Information and treat it as strictly confidential.  Associates should not discuss Fahrney-Keedy affairs with others

NOTES:

Accurately and safely prepares, administers, and documents the oral and/or topical medications that are commonly used in the facility and that may be ordered for resident use by the attending physician or the Medical Director.

 

 

Verifies that any medications brought into the facility by a newly admitted resident are examined and identified by the attending physician or the facility pharmacy.

 

 

Follows the labeling policies and practices of the facility.

 

 

Ensures that all medications administered are properly labeled.

 

 

Assists with medication charting as directed by the Nurse Supervisor or Charge Nurse.

 

 

Follows facility procedures for charting of medications.

 

 

Follows facility policies and procedures for destroying of medications; assists in carrying out these rules under the direction of the Director of Nursing, including documentation requirements.

 

 

Assists in documenting and removing medications that are discontinued by the attending physician.

 

 

Follows facility procedures governing drug receipt errors, including notifying the delivery source and the Director of Nursing.

 

 

Ensures that medication is returned and reordered except emergency drugs.

 

 

Ensures that documentation of the drug receipt error includes a written error report, signed by an observer/witness and you.

 

 

Follows facility procedures in holding medications for residents who are transferred from the facility or when the attending physician discontinues medications.

 

 

Accurately measures, records, and reports the vital signs of residents.

 

 

Observes and verifies that medication is ingested or applied as directed.  Documents any instance where prescribed medication is not administered including reason(s) for refusal.

 

 

Promptly notifies the Charge Nurse, Nurse Supervisor, or Director of Nursing of any errors or reactions to medications by residents.

 

 

Follows the medication card practices of the facility.

 

 

Ensures that no medication is administered without a written, signed, dated order from an attending licensed physician.

 

 

Follows the facility procedure for recording the order in the resident’s clinical record, including the type, route, dosage, frequency, and strength of each medication.

 

 

Verifies the identity of the resident before administering the medication treatment.

 

 

Assists the Nurse Supervisor or Charge Nurse in monitoring and documenting all aspects of such self-administering.

 

 

Promptly notifies the Nurse Supervisor or Charge Nurse of any failure of a resident to properly take any “self-administered” medications.

 

 

Follows established guidelines concerning the storage of drugs and biologicals.

 

 

Is familiar with the meaning and usage of common medical abbreviations, symbols, and terms relative to the administration of medications as used by the facility, pharmacists, and physicians.

 

 

Participates in the development and implementation of the resident’s comprehensive assessment, as necessary.

Notes and reports errors in the administration of medications.

 

 

Performs administrative requirements, such as completing necessary forms, charts, reports, etc. and submit these as may be required.

 

 

Maintains a friendly, productive, working relationship with other members of the department and facility.

 

 

Reports any complaints or grievances made by residents to the Director of Nursing.

 

 

Participates and cooperate with any facility surveys (inspections) made by authorized government agencies as requested by the Director of Nursing.

 

 

Agrees not to disclose assigned user ID codes and passwords for accessing resident/facility information and promptly reports suspected or known violations of such disclosure to the Director of Nursing.

 

 

Agrees not to disclose resident’s protected health information and promptly reports suspected or known violations of such disclosure to the Director of Nursing.

 

 

Reports any known or suspected unauthorized attempt to access the facility’s information system.

 

 

Ensures that departmental computer workstations left unattended are properly logged off or the password protected automatic screensaver activates within established policy guidelines.

 

 

Attends annual in-service training programs as scheduled.

WORKING ENVIRONMENT:

RESIDENT RIGHTS:

  • Maintains the confidentiality of all resident care information including protected health information.  Reports known or suspected incidents of unauthorized disclosure of such information. 
  • Ensures that you treat all residents fairly with kindness, dignity, and respect.
  • Ensures that all nursing care is provided in privacy.
  • Knocks and wait for permission to enter all residents’ rooms.
  • Reports all grievances and complaints made by the residents to the Director of Nursing.
  • Reports all allegations of resident abuse and/or misappropriation of resident property to the Fahrney-Keedy Abuse Coordinator.
  • Honors the resident’s refusal of treatment request.  Reports such requests to your supervisor.

 

SPECIFIC REQUIREMENTS:

  • Must be able to read, write, speak, and understand the English language.
  • Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
  • Must be a supportive team member, contribute to and be an example of teamwork and team concept.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with staff, residents, family members, visitors, government agencies/personnel, and the public.
  • Must possess the ability and willingness to work harmoniously with other staff.
  • Must have patience, tact, a cheerful disposition, and enthusiasm, as well as the willingness to handle difficult residents.
  • Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.
  • Must be able to relate information concerning a resident’s condition.

EDUCATION:

  • High School Diploma or GED preferred but not required.

 

 

PRIOR/RELATED EXPERIENCE:

  • Must be a Certified Medication Aide in accordance with the laws of this State.

 

 

PHYSICAL & SENSORY REQUIREMENTS (With or Without the Aid of Mechanical Devices):

  • Must be able to move intermittently throughout the workday.
  • Must be able to speak and write the English language in an understandable manner.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must be able to see and hear or use prosthetics that will enable these senses to function to ensure that the requirements of this position can be fully met.
  • Must function independently and have flexibility, personal integrity, and the ability to work effectively with residents, staff, and support agencies.
  • Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination.
  • Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility.
  • Must not have a weight-lifting restriction.  Must be able to push, pull, move and/or lift a minimum of twenty-five pounds to a minimum height of five feet and be able to push, pull, move, and/or carry a minimum distance of fifty feet.
  • May be necessary to assist in the evacuation of residents during emergency situations.
